Important Notice to Minister's Family Benefit Members
The below statement is from the IPHC Global Ministry Center in Oklahoma:
After a thorough review and analysis of the Minister's Family Benefit (MFB), it has been determined that changes are needed to make the program more financially sound. The last increase to the Contribution of Death was in 2000. Effective October 1, 2010, the Contribution of Death will increase to $5.00 for each death notification.
Minister's Family Benefit Program
Minister's Family Benefit is a life insurance program designed to benefit the ministers of the International Pentecostal Holiness Church. There are certain age limits that apply. For more information you may contact the International Pentecostal Holiness Headquarters in Oklahoma City, OK at 405-787-7110 and ask to speak with Minister's Family Benefit department.
To apply please print the application below and mail it to the Appalachian Conference, IPHC, Attention: Rhonda Smith, P.O. Box 1086, Dublin, VA 24084 with a $5.00 application fee per applicant.
